 By signing up to the Thrive mentoring scheme, I agree that I declare that the information I have provided is true and accurate to the best of my knowledge. I agree to commit the necessary time to engage with my mentor/mentee on a regular basis or to communicate with my mentor/mentee and the Career Mentoring Team should I no longer be able to dedicate the time required whether temporary or permanently. I agree to attend and comply with any compulsory sessions or evaluation requirements throughout the scheme. I agree that should I not meet the commitment expectations of the scheme that my mentor may be matched with another student. I agree to maintain confidentiality, in line with my responsibilities under the Data Protection Act, in relation to my mentor/mentees information or my mentor/mentees organisational information. I agree that I (as a mentee) will not share the mentee registration link to the Thrive platform with others, and understand this is for my use only as I have completed mentee training. I agree to the relevant personal information being shared with the mentor/mentee I am matched with. I agree to act in line with the mentee/mentor conduct expectations that will be outlined to me in the training. I agree to the terms of which my data may be shared following the submission of this form, as were outlined in the introduction.
